Pathway Plan reviews
We meet with you to review your Pathway Plan to make sure it is right for you. These meetings are very important.
When you were a child in our care, you had review meetings led by your Independent Reviewing Officer, these were important meetings to make sure the correct plan was in place for you.
As a care experienced young person you will still have a Pathway Plan and we need to catch up with you to make sure the plan is right for you and your needs.
How we review your Pathway Plan
When you're at one of your regular catch ups with your Your Young Person's Advisor (YPA), you will look together at a copy of your plan and agree if there need to be any changes. We use a document called My Plan, My Future to help make sure we note down all of your views, ideas and goals too. We also explore how we can support you to reach them.
We review the plan at least once every six months but we can do it more often if your plan needs to change.
You should have a copy of your plan and this should be written in a way that you can understand, whether this needs to be translated into another language or done in picture form.
Where and when we review your Pathway Plan
You can decide where you want to meet, this should be somewhere you feel comfortable and at a time that works for you.
Who will be at your Pathway Plan review
You and your YPA will arrange to get together but you might want someone else there with you, such as another professional working with you or someone important to you. You can decide who you want to be there.
When we have some worries about a young person's safety then a Team Manager or a Consultant Social Worker may also want to be there. We would, of course, ask you if you agree to this happening.
