Preparing for Adulthood (PfA) - outcomes across the age range 0-25 for children and young people with SEND

In County Durham, we have worked with partners, including young people, parents/carers and educational staff to adapt the national PfA outcomes toolkit.

This is to support schools, colleges and other post-16 providers in identifying the four key PfA pathways when developing outcomes in Education, Health and Care plans across the whole age range.
