This page tells you how to apply for Care Connect, our community alarm service.
How to apply

- contact Social Care Direct to Get your care needs assessed and find out if an alarm is appropriate for you
Urgent referrals
If you have an urgent referral outside of normal office hours (after 5.00pm during the week, or during the weekend or a bank holiday) please Contact Care Connect by phone and we will deal with your referral immediately.
What happens once you have applied
We will contact you to agree a suitable date to visit you in your own home, conduct a survey and to gather personal information. This is so that we are aware of your personal needs.
A monitoring alarm unit will then be installed and we will show you how to use it.

How much it costs
Our prices run from April to March each year. All prices include VAT.
All customers will receive a monitoring alarm unit and pendant which include the following charges:

VAT
No VAT is chargeable if a VAT exemption form is completed: Care Connect - Vat reliefs for disabled and older people (PDF, 101 KB)
If you are unsure whether you are eligible for VAT exemption, please contact the HM Revenues and Customs on 0300 123 1073 before filling this form in.
Cooling-off period
Should you decide that the service is unsuitable for you, after 21 days you can terminate your agreement and you will only pay our one-off connection charge.
